“Reasons why you should have akismet enabled” would sound better. It’s has become quite effective now.
Yes you’re right Akismet is quite effective now but I made a mock comment experiment on the other blog and still there are spam comments that goes through.
Don’t forget to check
Before a comment appears Comment author must have a previously approved comment
having that checked will ensure that the comments that get automatically approved are from people that you know.
Else, if you mean moderating every single comment; it’s not really recommended.
Why?
Because me (having comment notifier activated) can’t live with moderating every single comment. I need to attend classes, and my readers leave comments from time to time during the whole day when I’m unable to approve them. It would cut a *gap* between the discussion of my commenters on the blog.

Also if you can set the maximum number of links that should be in a comment , spammers tend to leave lots of links so better to set it to 1 or 2 and you can reduce lots of spam comments.
I also get to have problems on spam too. My blog is hosted in WordPress.com, so I get to have Akismet to help me moderate my blog. It is very effective, it just like the spam feature that free email services have.
I cannot turn on full comment moderation because it wastes time. I use “semi”-moderated settings for my comments: the first time commenter for the blog gets to pass moderation first, but once that the commenter’s comment is approved, subsequent comments are not anymore moderated.
Yeah there are alot of people that don’t like the comment moderation but I feel it’s a must because I get tons of spam and if you are trying to post a real valid comment then it shouldn’t really matter. That’s just the way I feel.

I think I have to set my comment moderation into semi-moderated setting as Gem pointed out, then allow only one or two links as Nishadha suggested, and of course check the “Comment author must have a previously approved comment” as Kevin reminded.

I have wordpress blog too and akismet really stopped those comment spams to come into my blog. Well, I had enough of spam comments even before that’s why I moderate my blog comments too…
Yup I like to moderate my own comment , akismet sometimes cought comment that are not spam i think.
